Some studies have raised concerns about testosterone therapy (TTh) increasing cardiovascular risk, but is the evidence really solid? This review critically analyzes those claims, revealing significant methodological flaws in studies linking TTh to higher cardiovascular risk. In contrast, numerous well-designed studies actually show that untreated low testosterone is linked to higher cardiovascular risk, while TTh can improve heart health when managed correctly.

So, if you've been hesitant about testosterone therapy because of heart health concerns, this study suggests that the risks may not be what they seem. Instead, proper patient selection and monitoring could make TTh a safe and beneficial option.

This week, someone asked me, “Hi Dr. Malik gm. I am having a problem, I am 62 years old and for some time now I have noticed that the skin of my penis cracks and turns red, I also urinate a lot. I went to my PC doctor and she gave me some ointment that worked for a time but the problem returns can you help?”. Here’s the answer to that, recurrent cracking, redness, and frequent urination could be signs of a few different issues like chronic skin irritation, a fungal or bacterial infection, or even an underlying condition like diabetes, which can lead to frequent urination and make the skin more prone to infections. It's great that the ointment helped temporarily, but since the problem has returned, it's important to take a closer look. I recommend talking to your urologist to see if maybe you should have a biopsy of the area or seeing a dermalogist to better evaluate the skin concerns. If you've been having a lot of wetness from urination on the skin, try some Vaseline to protect the skin from irritation from the urine. Sometimes baby butt cream can help heal areas of redness.

On a more personal note, my son is getting ready to rock the stage with a breakdancing routine for his local talent show, and I couldn't be more excited (and a bit nervous, I won't lie!). Seeing him prepare and perfect his moves has been an absolute joy, and I'm really looking forward to cheering him on.

I recently discussed a topic that many of my male patients are concerned about – getting an erection during a doctor's exam. Quote: "Getting an erection during an examination is not inappropriate or something to be embarrassed about."

Here are some quick takeaways

  • Getting an erection during an exam is a normal physiological response; it doesn't mean you're aroused.

  • It shows that your blood flow and nervous system are functioning well, which is a good health sign.

  • If it happens, stay calm and remember it's just your body's automatic response, and doctors understand it’s normal.

In this episode, I emphasized that "no doctor is gonna get mad, angry, upset, or accuse you of doing inappropriate things just because you got an erection during an examination."
